Please review all options below and decide which
fits best. If you hold a Gift Certificate from a previous camp we will
need to collect your Gift Certificate on day 1 as final verification.

Mites and Squirts - July 6-10 2009
$425.00 Pay in Full
This option pays for 1 participant in full for our annual Summer Camp held on July
6-10 2008
starting at 8:00 am and ending at 3:00 pm daily Mon-Fri. You will receive confirmation via e-mail,
a welcome letter with important information and a liability waiver to sign
and bring on day 1. Using this option speeds you through right through the sign-in on day 1.
Mites and Squirts - July 6-10 2009
$99.00 Deposit Only
This option places a Non-refundable $99 deposit insuring a spot for 1 participant for our annual Summer Camp held on
July 6-10 2009
starting at 8:00 am and ending at 3:00 pm daily Mon-Fri. Using this option the balance must be paid in full at
Sign in. You will receive confirmation via e-mail, a welcome letter with important information
and a liability waiver to sign and bring on day 1.
Mites and Squirts - July 6-10 2009
$807.50 Multiple Family Member Only
This option pays for 2 family member participants in full for our annual Summer Camp held on July 6-10 2009
starting at 8:00 am and ending at 3:00 pm daily Mon-Fri. Using this option you save 10% ($42.50) on one child. You will receive confirmation via e-mail,
and you will receive a welcome letter with important information and a liability waiver to sign
and bring on day 1.

$382.50 Gift Certificate Holders Only
This option pays for 1 participant who holds valid Gift Certificate for our annual Summer Camp held on
July 6-10 2009 starting at 8:00 am and ending at 3:00 pm daily Mon-Fri. Using this option
you save 10% of the regular cost because you attended
a previous Radius Edge Camp or Clinic. Your gift certificate information must then be confirmed at sign in so
please bring your certificate with you on day 1 and hand in at sign in. You will receive confirmation via e-mail,
and you will receive a welcome letter with important information and a liability waiver to sign
and bring on day 1. If you chose this option by mistake and do not hold a valid Gift Certificate you will
be required to pay the remaining balance on day 1 at sign in.

$382.50 MONARCHS HOCKEY PLAYERS
This option pays for 1 participant who is a MONARCHS HOCKEY PLAYER for our annual Summer Camp held on
July 6-10 2009 starting at 8:00 am and ending at 3:00 pm daily Mon-Fri. Using this option
you save 10% of the regular cost because you are part of the Monarchs Hockey Organization.
Your information will be confirmed at sign in. You will receive confirmation via e-mail,
and you will receive a welcome letter with important information and a liability waiver to sign
and bring on day 1. If you chose this option by mistake and are NOT a current member of the Monarchs Hockey Organization
you will be required to pay the remaining balance on day 1 at sign in.

This option pays for 1 participant who is a SALEM SAINT HOCKEY PLAYER for our annual Summer Camp held on
July 6-10 2009 starting at 8:00 am and ending at 3:00 pm daily Mon-Fri. Using this option
you save 10% of the regular cost because you are part of the SALEM SAINTS Hockey Organization.
Your information will be confirmed at sign in. You will receive confirmation via e-mail,
and you will receive a welcome letter with important information and a liability waiver to sign
and bring on day 1. If you chose this option by mistake and are NOT a current member of the SALEM SAINTS Hockey Organization
you will be required to pay the remaining balance on day 1 at sign in.
